In consideration for my attendance and participation in the martial arts training offered by Warrior Martial Arts Academy, I, the student/parent/guardian, acknowledge and understand that martial arts training is inherently dangerous and I knowingly and willingly assume all risk of injury or other damage associated with such training. On behalf of myself and my child, I further release Warrior Martial Arts Academy, its management, assigned staff, teachers, and fellow students from any claim of any and all liability that may result from any injury or damage received, whether for bodily injury or personal property, and I hereby waive all claims that I, my child, or anyone else on our behalves, may make with respect to such injury or damages. I also state that myself or my child is physically fit to take the prescribed course of instruction and do so of my own free will. In addition, I agree for myself, my child, and our successors that, should I, my child or our successors assert any claim in contravention to this agreement, I, my child and our successors shall be liable for the expenses including legal fees incurred by Warrior Martial Arts Academy, its management, assigned staff, and/or teachers in defending unless Warrior Martial Arts Academy, its management, assigned staff, and/or teachers are adjudged finally liable on such claim for willful and wanton negligence.

I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 or other diseases by attending the Academy and that such exposure or infection may result in injury, illness, disability, death, damage, loss, liability and/or expense of different kinds (collectively, the “Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, indemnify
and hold harmless the Academy, its employees and representatives, of and from any and all Claims. I understand and agree that this release includes any Claims based on the actions, omissions or negligence of the Academy, its employees and representatives, whether a COVID-19 infection occurs before, during or after participation in any Academy program. The foregoing is given in consideration for the Academy permitting me and/or my child(ren) to attend the Academy and participate in its programming.
